Criteria for Volunteer Pilots:

1. 21 years of age or older
2. Must be current with a minimum of 250 hours of pilot in command time
3. Submit a completed and signed application;
4. Provide proof of insurance with a copy of the declaration or certificate page
5. Send a copy of most recent log book entry for BFR or wings phase
6. Provide updated information whenever reportable changes occur such as loss of privilege, contact or aircraft insurance information

Pilot Responsibilities:

As a LifeLine Pilot, you will receive 3 forms by fax or email after you accept a mission:

Proposed Mission Plan - If you need to alter the Proposed Mission Plan, feel free to do so but PLEASE notify the mission coordinator immediately so we are fully informed.

Day of Mission: Flight Authorization, Release of Liability and Indemnity form (2 pages) - to be filled out BEFORE the flight by both the patient and pilot and faxed or mailed to our office PRIOR to the flight.

Mission Follow-Up Form - to be filled out by you AFTER the flight and faxed/mailed to LifeLine Pilots.

If there are any problems with your mission, PLEASE CONTACT LIFELINE PILOTS by calling the after hours phone number, (the number is located on the Mission Plan) and indicate any changes or problems on your Follow-up Form.  Your comments help us improve our procedures and maintain accurate records.

Call the patient/passengers at least 24 hours prior to flight to verify the names and number of passengers going, departure airport and time.  If any of this differs from the Proposed Mission Plan, please contact a mission coordinator.

If the flight is more than one leg, call the link-up pilot to verify/coordinate time and place of link-up.

Patients/passengers are informed that cancellations occur due to weather, etc. If you cancel, please notify both the patient/passenger and mission coordinator immediately.

Safety is of the utmost concern for every mission.  LifeLine Pilots would rather cancel than have a pilot fly in a potentially dangerous situation.

Each volunteer LifeLine Pilot...

Will wear their LifeLine Pilots' name badge and cap so that they are easily identified by flight recipients and passengers. As a LifeLine Pilot, you will receive your first cap free of charge. Additional caps may be purchased and charged to your credit card by calling the office, 309-697-6282.

Will provide a report describing the circumstances of any aviation accident or incident, regardless of whether on a mission or a personal flight, to LifeLine Pilots. This will be reviewed by the Safety Committee.

Agrees that they will not charge a fee, accept an honorarium or donation from any passenger or other party wishing to reimburse the Pilot for expenses incurred as a result of any LifeLine Pilots' flight.

Wishing to remain an active pilot, will respond to the annual Renewal Letter and provide updates for the database.

LifeLine, Inc. dba LifeLine Pilots, has been granted tax exempt 501(c)(3) status by the IRS. All contributions are deductible to the fullest extent of the law. LifeLine Pilots operates solely on the contributions of private citizens, various foundations/corporations and donated aircraft and pilot expenses.
